You can print this form out and bring it to your child's doctor to sign. The original plus two copies should be returned to the Booster secretary along with the signup form. Keep in mind that no child will be allowed to play sports without this form on file. Each coach is required to have a copy of this form with them at all times for the child's safety and the parents well being. Note: If the original physical form and two copies are submitted AFTER practices have begun, please send original and one copy to Booster secretary and give a copy to your coach.
