School Nurse

Saint Rita School employs a full-time Nurse who protects and promotes our students' health and safety. She provides first aid care and health assessment, communicates with teachers and staff to ensure the safety and well-being of students, and coordinates and provides vision screenings.

For more information on screenings, allergies, and administration of medicine, please click on the forms below. If there are any questions regarding the health forms, please contact the school nurse. All forms returned to the school require a parent signature.

Prescriber and Parent Request for the Administration of Medication at School: This form is necessary for any medication to be administered to a student during school hours. A physician signature is required for prescription medication.

Allergy Action PlanThis form is to be used if the student has any allergies. It must be completed and a physical signature is required.

Asthma Action Plan: This form is to be used for a student with asthma. It should be completed and a physical signature is required.

Physical Examination Form: This form is to be submitted prior to student enrollment.
